About The Position

Helio Health is seeking a Supportive Housing Case Manager to provide case management services to residents of their new Supportive Housing program, Confluence Court, in Binghamton, NY. This role will support up to 30 residents receiving ongoing care for Substance Use Disorder, working alongside a Team Leader and Peer Specialists. The program is set to open in August 2025, and the hiring process for a Team Leader is starting immediately to promote the program and accept referrals. The position will initially be remote and transition to on-site work once the program opens.

Requirements

  • High School Diploma or GED required.
  • Valid NYS Driver’s License required.

Nice To Haves

  • Associate's degree in a related human services field preferred.
  • One year of experience working with behavioral health and/or substance abuse issues preferred.

Responsibilities

  • Maintains a case load and performs primary case work responsibilities including: Screening, intake, orientation, referrals, counseling, crisis intervention, patient/resident education, case management, reporting and record keeping, consultation with other professionals, coordination of care with other community providers, and discharge planning.
  • Trains staff, volunteers, and students/interns.
  • Performs Quality Assurance activities.
  • Provides services according to agency policies, procedures, program guidelines, and governmental regulations.
  • Monitors provision of services when assigned.
  • Evaluates programs and identifies strengths/weaknesses.
  • Assists in problem resolution and implementation of supervisor approved program modifications.
  • Participates in program administrative and clinical committees as assigned.
  • Represents agency to the community.
  • Other duties as assigned by supervisor.
