About The Position

As a Supported Living Assistant Manager at Pathfinder, you play a key leadership role in ensuring individuals receive safe, consistent, and person-centered supports in their homes. By supervising teams, coordinating care, and supporting daily operations, you help create stable environments where people can thrive and live meaningful, independent lives. Requirements

  • Minimum of a high school diploma/GED.
  • Prefer 2 years experience in the field of Human Services.
  • At least 18 years of age.
  • Basic language, written, and computer skills necessary for communication and documentation.
  • Demonstrated ability to support people with disabilities.
  • Ability to work a flexible schedule if needed.
  • Crisis prevention, intervention, and teaching skills.
  • Valid driver’s license, insured vehicle, and safe driving record.
  • Ability to speak, hear, and read the English language.

Responsibilities

  • Provide direct oversight and support to Direct Support Professionals (DSPs) across assigned residential sites
  • Serve as the primary point of contact for DSP scheduling needs and coverage concerns
  • Lead team meetings to reinforce expectations, communication, and person-centered practices
  • Monitor staff training compliance and model ethical, effective leadership
  • Support the implementation of individualized support plans (PCISPs) in collaboration with the Supported Living Manager
  • Coordinate medical appointments, day program participation, and transportation needs
  • Participate in PCISP, quarterly, and leadership meetings as assigned
  • Ensure homes meet safety, health, and quality standards at all times
  • Maintain accurate case notes, billing records, and Medicaid Waiver documentation
  • Oversee medication administration, documentation, and refill processes
  • Identify and report maintenance and operational needs promptly
  • Act as a liaison between families, guardians, DSPs, and leadership
  • Communicate staffing or operational concerns to the Supported Living Manager in a timely manner
  • Collaborate with leadership and on-call teams to ensure shift coverage when needed
  • Ensure individuals receive consistent, high-quality, person-centered supports
  • Support and develop DSP teams through guidance, structure, and leadership
  • Strengthen communication and continuity of care across residential services
  • Advance Pathfinder’s mission by promoting dignity, safety, and empowerment
