About The Position

This position is responsible for the high-quality delivery and coordination of the agency’s comprehensive supportive services, including workforce development, academic support, and the Day Services Program. The Coordinator provides ongoing support and expertise through the assessment, planning, implementation, and outreach of both residential and drop-in community youth. The overall goal of the position is to enhance the quality of the Community Service Center services, manage crises, and provide continuity of care through the integration of outreach, case management, and discharge planning. By supporting all programs in carrying out these day-to-day responsibilities, this role helps fulfill the mission of Covenant House Georgia. Requirements

  • Bachelor’s degree in Human Services or a related field required, though an equivalent combination of education and experience (e.g., Associate’s Degree with a minimum of 5 years’ experience, or a High School Diploma with 10 years’ experience) may be considered.
  • Minimum of 2 to 3 years of experience working with at-risk youth required.
  • Minimum of 3 years’ experience working in a residential setting desired.
  • Knowledge of mental health and substance use is required.

Responsibilities

  • Collaborate with CHGA staff to cultivate, implement, and maintain new corporate and community partnerships.
  • Facilitate communication with external workforce and academia partners to maintain up-to-date training curricula and pipelines to support youth.
  • Identify, build, and maintain social enterprise and certification programs.
  • Develop knowledge of current community resources and referrals, and refer clients for services outside the role's scope of practice.
  • Collaborate with the Community Engagement Program Manager to support CHGA’s Youth Advisory Board, CHI’s Youth Congress, and the CoC’s Youth Committee.
  • Complete intakes, meet the immediate needs of drop-in youth, and support Drop-In center clients when needed.
  • Develop individualized case plans with measurable goals based on identified needs and complete housing assessments for all unhoused youth.
  • Oversee the day-to-day flow of day services and maintain a robust monthly planner for services rendered in the Lifelong Learning Center.
  • Ensure enriched programming related to physical and emotional wellness, life skills, workforce development, academic support, and youth advocacy.
  • Conduct all youth interactions in a manner that promotes acceptance, respect, honesty, and emphasizes CHGA’s Five Principles.
  • Assist with the planning and delivery of morning outreach services.
  • Ensure the integration of social services and case management functions into family reunification, discharge, and housing processes with external service organizations.
  • Provide and encourage a continuum of care approach for CHGA youth throughout various service programs.
  • Ensure compliance of files in accordance with specific city, state, and federal guidelines, entering data into Efforts to Outcomes (ETO) and Client Track in a timely manner.
  • Provide weekly review of record keeping for Drop-In Center clients to ensure documents are completed and filed within the requested timeframe.
  • Participate in weekly or bi-weekly case reviews, support panels, and multidisciplinary team meetings to support the development of individualized care plans.
  • Monitor and evaluate program effectiveness, discussing observations with supervisors to maintain and enhance program functioning.
  • Participate in organizational processes, such as policy review and risk management, and assist in the development of community partnerships.
  • Establish and maintain effective, ethical working relationships with co-workers, partnering organizations, representatives of other agencies, and the public.
  • Uphold Covenant House Georgia standards of conduct, policies, and procedures, modeling appropriate, professional behavior to maintain a safe and inviting atmosphere.
  • Oversee all safety concerns for the day program, Lifelong Learning Center (LLC)/drop-in, and residential youth, ensuring all buildings and surrounding areas are safe.
  • Rover the property for safety and ensure that all youth are in their assigned groups.
  • Lead community and assembly meetings.
  • Ensure that all necessities have made it to the gym, including shuttle keys, lunches, and Marta cards.
  • Transition all critical information between overnight, day, and second shifts, including monitoring needs for specific youth.
  • Provide crisis intervention independently, make informed decisions for resident infractions, and respond immediately to emergencies at the facility.
  • Serve as the on-call Manager on Duty during assigned week rotation to respond to crisis calls, emergency calls, and other agency needs.
  • Immediately report all maintenance concerns to leadership and follow all department procedures for resident schedules, shift transitions, and food preparation.
