Requirements

  • Experience supporting adults with intellectual or developmental disabilities
  • Strong communication, organization, and time management skills
  • Ability to manage multiple priorities and maintain consistent scheduling
  • Proficiency with Microsoft Office and documentation systems
  • Valid Connecticut driver’s license required
  • Cultural sensitivity and ability to work with diverse populations

Nice To Haves

  • Bachelor’s degree in Human Services or related field preferred, or 2+ years relevant experience

Responsibilities

  • Develop, update, and implement individualized service plans and annual goals
  • Facilitate and prepare for annual review meetings with Community Members and providers
  • Meet regularly with assigned individuals to provide structured support
  • Provide coaching in daily living skills including budgeting, meal planning, and home organization
  • Coordinate healthcare appointments and support follow-through on care plans
  • Assist with financial skills such as budgeting, banking, and bill payment systems
  • Support community engagement and development of social supports
  • Maintain accurate documentation, case notes, and reports in Setworks
  • Advocate for appropriate services and collaborate with interdisciplinary teams
