Supervisory Social Worker- Community Nursing Home

Veterans Health Administration•Orlando, FL

About The Position

The Supervisory Social Worker supervises the Community Nursing Home (CNH) Program, assisting with planning, establishing, and implementing policies and procedures; developing program goals and objectives; and monitoring, operating, evaluating, coordinating, and overseeing program staff. Alternate locations may be negotiated based on space availability. The position works closely with Community Health Nurses assigned to CNH and CCN SNF Programs and Community Care leadership to ensure compliance with VHA directives, TJC standards, state licensure standards, and VA-OIG expectations. It conducts ongoing reviews and appraisals, provides technical assistance to public, state, local, private, and university partners, and performs program analysis, development, review, research, design, planning, monitoring, and assessment. Work may occur at locations away from the main unit where day-to-day supervision is difficult. As part of an interdisciplinary treatment team, the Social Worker develops and updates Veteran-centered treatment and discharge plans with Veterans and, when possible, their families. The role includes reassessing needs with service providers, educating Veterans and caregivers about available services, arranging referrals according to Veteran or surrogate preferences, and developing crisis management plans. In emergencies, staff follow Orlando VA Healthcare System protocols, including suicide risk assessments and Suicide Behavior and Overdose Reports. The incumbent independently implements evidence-based treatment modalities and provides individual, group, or family psychotherapy, supportive contact, and psychosocial education. The Social Worker uses standardized instruments, including Measurement-Based Care, to assess mental health problems and address psychosocial stressors. The position participates in assigned meetings, team huddles, interdisciplinary team meetings, committees, and Social Worker and matrix staffing meetings. Work outside the assigned area requires supervisor approval. Other duties include contributing to clinical reminders and facility performance measures within the Social Work scope of practice and privileges. The work schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. Ad hoc telework is available; this is not a virtual position. Relocation and recruitment incentives and permanent change of station are not authorized.
