About The Position

Human Touch Home Health is looking for an experienced and dependable Registered Nurse (RN) to provide PRN supervisory oversight for our non-skilled/personal care services throughout Washington, DC. This is not a traditional skilled nursing visit position. We're looking for an RN who understands the importance of client safety, caregiver supervision, accurate care plans, regulatory compliance, and consistent quality of care in the home. Requirements

  • Active Washington, DC Registered Nurse License Required
  • Graduate of an accredited nursing program
  • Previous nursing experience required
  • Excellent communication and documentation skills
  • Strong organizational and time-management abilities
  • Comfortable working independently in clients' homes
  • Reliable transportation and ability to travel between client locations throughout Washington, DC

Nice To Haves

  • Experience with personal care, private duty, non-skilled home care, home health, or community-based services preferred
  • Strong assessment and supervisory skills
  • Knowledge of care planning and caregiver oversight

Responsibilities

  • Conduct initial and ongoing client assessments as required
  • Complete supervisory visits in clients' homes
  • Develop, review, and update client plans of care
  • Assess clients for changes in condition or care needs
  • Ensure caregivers are following established care plans
  • Observe and evaluate caregiver performance in the home
  • Provide education, coaching, and guidance to caregivers
  • Address client and family concerns related to care
  • Communicate changes in client needs to the appropriate team members
  • Help ensure services are being delivered safely and appropriately
  • Review documentation for accuracy and completeness
  • Assist with quality assurance and regulatory compliance
  • Maintain timely and accurate clinical documentation
  • Collaborate with scheduling, operations, and leadership to resolve care concerns
  • Escalate significant changes or concerns when appropriate
