Supervisor-Integrated Health and Social Care
About The Position
The Supervisor-Integrated Health and Social Care supervises a team of community health workers, provides direct Community Health Worker services, and assists the community health leader in core community health department functions. A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service standards.
Requirements
- Bachelor's degree in community health, public health, or other with related coursework
- 3 or more years CHW experience or equivalent
- Certified in at least one chronic disease certification program (i.e. DPP)
- Basic Life Support
- Strong relationship and collaboration skills
- Excellent communication skills
- Ability to work independently
- Good leadership and team management skills
- Compassion for poor and underserved members of the community
- Epic electronic medical record, reporting, dashboards
Nice To Haves
- Community Health Worker Certification if available
- Additional language skills as applicable to community
Responsibilities
- Trains, monitors, and evaluates CHWs according to performance standards.
- Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
