About The Position

Make a lasting impact at one of Portland's most innovative programs focused on ending homelessness. The Blackburn Center brings together housing, primary care, behavioral health, substance use treatment, employment services, and recovery support under one roof, creating truly integrated, client-centered care for some of our community's most vulnerable populations. As a Mental Health Clinician, you'll join a multidisciplinary team providing trauma-informed, whole-person care to individuals experiencing complex mental health, substance use, medical, and housing challenges. You'll conduct assessments, develop treatment plans, provide individual and group counseling, support crisis intervention efforts, and collaborate closely with medical providers, case managers, housing teams, and community partners. This role is ideal for clinicians who are passionate about serving underserved populations, believe in harm reduction and recovery-oriented care, and want to work in an environment that values collaboration, innovation, cultural responsiveness, and continuous improvement. Requirements

Must meet qualifications for: QMHP or QMHP-R
LPC-A or LPC
CSWA or LCSW
CADC-R, at minimum, certification preferred, or the ability to obtain one within 1 year.
Has the competencies necessary to: Identify precipitating events
Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts
Assess family, social and work relationships
Conduct a mental status assessment
Document a multiaxial DSM diagnosis
Write and implement or supervise implementation of a treatment plan
Conduct and document a mental health assessment
Provide mental health treatment and rehabilitative services within ones scope of practice
Must meet CCC privileging requirements as required by FTCA.
Must have current CPR certification prior to start.
If using a vehicle for agency use: Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy; pass an initial drivers training within 60 days of being an approved driver and continued recertification training; and maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
Must pass a pre-employment drug screen, TB Test, and background check.
This includes clearance by the DHS Background Check Unit.

Must become a certified Director's Custody designee in Multnomah County within the first 6 months of hire.

Responsibilities

Develop collaborative treatment plans with clients and interdisciplinary care teams.
Partner with care teams to ensure client safety and continuity of care.
Coordinate services with healthcare providers, housing programs, community agencies, and family supports.
Support individuals often facing the greatest barriers to care and stability.
Promote dignity, autonomy, and self-determination in every client interaction.
Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered, and trauma informed integrated services.
Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
Proactively identify opportunities for improvement within Blackburn Center workflows and logistics.
Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested.
Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent.
Conduct and document Mental Health Assessments, including mental status exam, DSM diagnosis, and clinical formulation.
Develop care plan with client with assistance & input from all of the clients care team providers
Participate in outreach activities necessary to offer services and engage participation in a non-coercive manner.
Escort clients to meetings and appointments as necessary.
Participate in evaluating clients for risk of harm & initiate Directors Custody transport holds when appropriate.
Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients.
Collaborate with Residential facilities, Adult Foster homes or other housing providers to provide necessary treatment services and coordinate residential and non-residential treatment and service planning.
Participate in admission and discharge planning for any client needing hospitalization or subacute level of care.
Client contact must be made within one working day of admission.
Monitor and assure that the health and safety needs of clients are met.
Maintain client records using an electronic medical record.
Must be documented as required by program standards and funding requirements including documentation of clinical interactions, behavioral health assessments & individual service plans.
Participate in regular supervision from the Clinical Supervisor, seeking additional supervision whenever necessary.
Attend all mandatory CCC trainings in a timely manner.
Conduct educational and therapeutic groups for clients, using approved curriculum.
Coordinate services with all members of client's support network.
Participate in quality assurance and quality improvement activities as directed.
Assist with the development of an integrated treatment program.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access.
Perform other duties as assigned.
