About The Position

The Community Resource Center (CRC) is a specialized department at SAC Health focused on addressing community health inequities such as limited access to care, food insecurity, and housing challenges in the San Bernardino region. All CRC staff require a foundational understanding of Social Determinants of Health and their impact on underserved populations. Reporting to the Manager of the Community Resource Center, the Supervisor will oversee staff activities to ensure efficient patient services and effective planning of community events. This role involves providing direct supervision and guidance to CRC staff in areas such as intake, registration, patient flow, and referrals. The Supervisor will also assist in the daily operations of the department, with a particular focus on overseeing CRC programs and events. Key duties include organizing and managing the information and resources provided by staff, collecting and analyzing data, and communicating findings to achieve departmental objectives as set by the department head.

Requirements

  • AA Degree in related field required.
  • Valid California driver's license, and auto insurance.
  • Must receive EPIC certification for the module you have been hired into.
  • Minimum 3 years of supervisory experience required.
  • Minimum 3 years of experience working with the community and developing health and social work related programs required.
  • The ability to use project planning software, spreadsheets, and word processing software; input data, typing, copying, and effective use of the telephone.
  • Experienced with Microsoft Outlook, Excel, PowerPoint, and Publisher.
  • Strong planning, problem solving, documentation, and organizational skills.
  • Ability to analyze and interpret data, analyze and interpret processes and needs; define problems and solutions.
  • Able to manage time effectively and plan and implement objectives effectively.
  • Ability to maintain a professional attitude in difficult situations and communicate effectively.
  • Understands needs of clinic and works well in a team environment.
  • Must have the ability to be assertive.
  • Demonstrate leadership skills.
  • Ability to balance administrative responsibilities, client needs and task management skills are required.

Responsibilities

  • Oversees the activities of the Program Coordinators and CRC staff to ensure prompt and courteous patient services.
  • Monitors the day-to-day flow of information within the department, and flow of program demands to ensure consistency and quality client-centered services.
  • Provides support to staff in the absence of program manager.
  • Provides general supervision and direction for CRC staff and monitors patient flow, patient check in, and resolves patient issues.
  • In partnership with CRC Manager, assists in the recruitment, hiring, and training of staff, and manages staff performance in accordance with department and organization policy.
  • Coordinates case conferences, department and multi-disciplinary team meetings as needed.
  • Assesses the needs of the work team to determine the efficiency and effectiveness.
  • Ensures staff completes proper documentation within specified time-frame as stated in policy.
  • Reviews staff case notes for accuracy and detail, on a regular basis.
  • Assists with interviewing and hiring new candidates.
  • Works effectively and builds relationships with all departments.
  • Assists the CRC Manager with reviewing and implementing departmental protocol changes.
  • Assists the CRC Manager in identifying program grants for the department.
  • Works with Operations Project Manager and Grant Writer to identify grants and donations to serve the community.
  • Oversees scheduling of employees, evaluates performance, resolves patient and staff complaints.
  • Coordinates registration operations as strategically defined by the CRC Director or Manager.
  • Position requires driving between clinic locations; must have a reliable vehicle, valid driver's license, and auto insurance.
