About The Position

The Supervisor, Certified Clinically Advanced Nutrition Coordinator (CCANC) acts as an integral part of the care team, supporting physician-led practice areas by driving clinical excellence, team-based care, evidenced-based medical nutrition therapy, and clinical and emotional support to patients and caregivers. The Supervisor, CCANC formulates cohesive relationships with primary and consulting providers, nursing professionals, social workers, case managers, pharmacists, practice leadership, and office staff to foster the ongoing delivery of medical nutrition therapy, patient education, and clinical care to support optimal treatment outcomes. In collaboration with the medical team, the Supervisor, CCANC creates customized care plans for the provision of appropriate nutrition/nutrition support. The Supervisor, CCANC coordinates care with interdisciplinary team members to support smooth transitions between hospital and home. The Supervisor, CCANC adheres to department and hospital philosophies, standards, objectives, and policies and procedures.

Requirements

  • Bachelor’s degree or higher from an accredited institution, with completion of ACEND (Accreditation Council for Education in Nutrition & Dietetics) - accredited dietetic internship
  • Registered Dietitian (RD) from the Commission on Dietetic Registration (CDR) required
  • Advanced Certification, i.e. Certified Diabetes Educator (CDE), Certified Nutrition Support Clinician (CNSC), and/or Pediatric/Renal/Oncology Specialty required
  • 10+ years of clinical experience
  • 2-3 years in specialty area
  • Demonstrated program coordination experience and ability to communicate and work effectively with a multi-disciplinary medical team.

Nice To Haves

  • Master’s Degree preferred
  • Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions preferred
  • Research experience preferred

Responsibilities

  • Successfully facilitates the nutritional management of patients in clinical practice area in both in and outpatient settings.
  • Actively engages with physicians, nurse practitioners, physician assistants, nurses, social workers, pharmacists, administrative staff, and other health care professionals.
  • Assists with coordination of care, discharge planning, and transitions of care.
  • Conducts comprehensive assessments, providing patient and family/caregiver education, and executing care plans with managing physician to optimize nutritional status as it relates to the patient's treatment.
  • Uses professional judgment to adapt and apply guidelines, protocols and professional standards of care to the new and changing needs of the patient.
  • Collaborates with service providers and the primary team to ensure patient and caregiver understanding of the treatment plan.
  • Communicates weights, appropriate lab values, symptoms, side effects of medications / treatment, and clinical status to the managing provider(s) and care team.
  • Follows-up with the patient and caregiver to communicate treatment plan updates.
  • Documents patient encounters as per departmental protocols.
  • Ensures clear documentation of clinical process measures related to treatment.
  • Supports patient goal setting and documents the patient’s progress in achieving their goals.
  • Communicates visit findings, including urgent care matters, with managing physician, consulting and primary care team in a timely manner.
  • Supports the training and development of nutritionist team members, providers, and clinicians.
  • Builds cohesive relationships with providers, nurses, pharmacists, nutritionists, and social workers to support the multidisciplinary approach to care delivery.
  • Provides training and development of evidenced based practice in clinical focus area.
  • Supports ongoing improvement and refinement of skills within clinical specialty through feedback and recommendations.
  • Identifies opportunities to enhance the Service/Program.
  • Identifies variations in patient care practices, and nutrition or clinically related concerns at the practice level.
  • Develops strategies to improve patient care, and works with practice leadership to build and implement new standard operating procedures to enhance clinical care.
  • Participates in research activities.
  • Participates in the recruitment, hiring, training, and development of department staff.
  • Participates in the evaluation and disciplinary processes of department staff.
  • Supports department/program goals.
  • Ensures compliance with department /hospital policies and procedures, quality standards, and safety standards.
  • Works rotating weekends and holidays, as needed.
