About The Position
The Supervisor, Certified Clinically Advanced Nutrition Coordinator (CCANC) is an integral part of the care team, supporting a physician-led practice area by driving clinical excellence, team-based care, evidenced-based medical nutrition therapy, and clinical and emotional support to patients and caregivers. The Supervisor, CCANC forms cohesive relationships with primary and consulting providers, nursing professionals, social workers, case managers, pharmacists, practice leadership, and office staff to foster the ongoing delivery of medical nutrition therapy, patient education, and clinical care to support optimal treatment outcomes. In collaboration with the medical team, the Supervisor, CCANC creates customized care plans for the provision of appropriate nutrition/nutrition support. The Supervisor, CCANC coordinates care with interdisciplinary team members to support smooth transitions between hospital and home. The Supervisor, CCANC adheres to department and hospital philosophies, standards, objectives, and policies and procedures.
