PROGRAM Substance Use Disorder Services (SUDS) is a program designed to provide assistance to teens under probation supervision. Teens may be experimenting or using alcohol or drugs, but have no previous history or treatment experience. This position works as a partner with the Juvenile Probation Department. Work may be done at Probation offices, an assigned courthouse or meeting with clients in the community. POSITION Provide case management for an active caseload of twenty-five to thirty (25-30) youth/wards and interfacing between Juvenile Probation Department and the Community Adolescent Treatment Programs, in the establishment of ancillary client services. The SUD Case Manager is responsible for assessment, intervention plan development, and referral services. Conduct observed collection of urine samples for testing, follow-up and monitor youth/wards cases while enrolled in SUDS. Conduct ongoing home, school and treatment program visits with youth/ward, family, treatment providers and school teachers. Participate in the team meetings, compile and report monthly progress reports to Probation. Maintain case files on all participants. Enter data in the PCMS database and provide end of the month stats to the data entry/admin staff.
