About The Position

All In Solutions is seeking a dedicated Case Manager for its outpatient substance use disorder and mental health program. The Case Manager serves as a link between clients, treatment providers, and community resources, providing assessment, planning, advocacy, and care coordination to support client stability and long-term recovery. The role works collaboratively with clients and clinical staff to develop individualized service plans, monitor progress, and connect clients with needed services. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., in Eatontown, New Jersey.

Requirements

• At least 1 year of experience in case management or a behavioral health setting.
• Strong knowledge of substance use disorders and co-occurring mental health issues.
• Proficiency in documentation and electronic health records.
• Ability to listen actively, show empathy, and communicate effectively with individuals from diverse backgrounds.
• Genuine desire to support individuals in recovery and commitment to their well-being.

Nice To Haves

• Bachelor’s degree in psychology, counseling, social work, or a related field.
• Relevant certification or licensure.

Responsibilities

• Conduct initial and ongoing assessments of clients’ psychosocial, environmental, and financial needs.
• Develop and implement individualized service plans in collaboration with clinical staff and clients.
• Coordinate services with internal departments and external agencies, including housing, employment, legal aid, and medical care providers.
• Advocate for clients to ensure access to appropriate treatment and support services.
• Maintain detailed and timely documentation in accordance with regulatory and organizational standards.
• Monitor client progress and adjust plans as needed to promote recovery goals.
• Participate in case conferences, clinical team meetings, and training sessions.
• Provide crisis intervention and problem-solving support when needed.
