About The Position

The Substance Use Disorder (SUD) Specialist at Community Health & Wellness Partners of Logan County is responsible for providing comprehensive behavioral health services to individuals experiencing substance use disorders. The role involves assessing, planning, and implementing treatment plans to support patients in their recovery journey. The specialist will work collaboratively within a multidisciplinary team to deliver integrated care that addresses the complex needs of patients, promoting holistic wellness and recovery.

Requirements

  • Degree in chemical dependency counseling (CDCA) or related field.
  • Minimum of one (1) year of experience in behavioral health treatment, substance use treatment, or integrated primary care practice.
  • Experience in developing and implementing individualized treatment plans for clients with substance use disorders.
  • Proven ability to work collaboratively within a multidisciplinary team to provide comprehensive care.
  • Demonstrated experience in conducting assessments and providing evidence-based interventions for individuals with substance use disorders.
  • Strong understanding of substance use disorder treatment modalities and recovery principles.
  • Excellent communication skills, both verbal and written, with the ability to effectively engage and motivate clients.
  • Ability to maintain confidentiality and adhere to ethical standards in all professional interactions.
  • Proficient in the use of electronic health records and other relevant technology for documentation and communication.
  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines.
  • Demonstrated ability to work independently and as part of a team in a fast-paced environment.
  • Commitment to continuous professional development and staying current with best practices in substance use disorder treatment.

Nice To Haves

  • Active Counselor License within the State of Ohio, or actively pursuing licensure.
  • LCDC II, LCDC III, LICDC, LICDC-CS, LPC, LPCC.
  • Chemical Dependency Counselor License, preferred.

Responsibilities

  • Conduct thorough assessments of patients with substance use disorders to determine the appropriate level of care and treatment needs.
  • Develop individualized treatment plans that incorporate evidence-based practices and are tailored to meet the specific needs of each patient.
  • Provide direct counseling and therapeutic interventions to individuals and groups, utilizing a variety of treatment modalities to support recovery.
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate care and ensure comprehensive treatment for patients.
  • Monitor patient progress through regular follow-ups and adjust treatment plans as necessary to achieve optimal outcomes.
  • Maintain accurate and up-to-date documentation of patient interactions, treatment plans, and progress notes in compliance with organizational and regulatory standards.
  • Educate patients and their families about substance use disorders, treatment options, and strategies for maintaining recovery and preventing relapse.
  • Participate in case conferences, team meetings, and continuing education opportunities to enhance professional development and stay current with best practices in substance use disorder treatment.
  • Advocate for patients' needs and rights within the healthcare system and community resources.
  • Engage in outreach and community education efforts to raise awareness about substance use disorders and available treatment services.
  • Support the development and implementation of program initiatives aimed at improving the quality and effectiveness of substance use disorder services within the organization.
