Requirements
- One year of experience in social services field.
- Bachelor’s degree in Social Work, Counseling, Psychology, or related field.
- Experience working with individuals with Substance Use Disorders.
- Case Management experience.
- Knowledge of SUD treatment and recovery principles.
- Strong communication, organizational, and advocacy skills.
- Ability to work effectively within a multidisciplinary team.
Nice To Haves
- Applicants with experience in a correctional health care setting will be given preference.
- Certification or credential in substance use counseling (e.g., CADC, CSAC, or equivalent) strongly preferred.
Responsibilities
- Provides access to community programs.
- Ensures continuity of care following release into the community.
- Advocates for patient needs.
- Acts as a liaison between the multiple systems impacting the lives of offenders with mental health illnesses, co-occurring substance use disorders, and chronic medical illnesses.
