Substance Use Disorder Healthcare Navigator
About The Position
Southwest Virginia Community Health Systems (SVCHS) is seeking a Substance Use Disorder (SUD) Healthcare Navigator to join their New Day Recovery Program in Saltville, VA. This role supports individuals navigating recovery from substance use and co-occurring mental health conditions. The Healthcare Navigator guides patients through the continuum of care by coordinating services, removing barriers, and connecting individuals to internal SVCHS programs and external community resources. This position requires strong relationship-building skills, patient advocacy, and knowledge of recovery-oriented systems of care.
Requirements
- High school diploma or equivalent (required)
- Experience in community health, healthcare navigation, case management, behavioral health, or social services
- Understanding of Substance Use Disorders (SUD), mental health conditions, and community-based resources
- Valid driver’s license with the ability to transport patients as needed
- Strong communication and interpersonal skills
- Ability to build trust with diverse and vulnerable populations
- Effective care coordination and problem-solving abilities
- Team-oriented with the ability to work in a fast-paced environment

- Background in community health, case management, behavioral health, or peer recovery support
- Passionate about improving access to care and addressing social determinants of health
Responsibilities
- Coordinate access to Substance Use Disorder (SUD), behavioral health, and primary care services within SVCHS and the community
- Develop, maintain, and strengthen referral pathways with community partners (housing, employment, mental health, social services)
- Provide care navigation, follow-up, and patient engagement support to ensure continuity of care
- Assist patients in overcoming barriers to care, including transportation, insurance, and social determinants of health
- Support and educate individuals participating in peer recovery services
- Maintain up-to-date knowledge of local and regional community resources
- Document patient interactions and care coordination activities in accordance with organizational standards
