ESSENTIAL DUTIES AND RESPONSIBILITIES to be learned while performing the duties of a Substance Use Disorder Trainee. Other duties may be assigned. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served. Demonstrates knowledge of the principles of growth and development over the life span. Be able to identify and categorize each patient’s age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients. Provide screening, evaluation and referrals of individuals to other treatment/service programs that are available in the community. Provide individual and group counseling for persons in the process of discovering and dealing with drug related problems and emerging mental health issues. Serves as the co-case manager for a group of up to ten youth. Implement problem-oriented records system, Federal/State/County reports, patient surveys, and submits on a monthly basis to Supervisor. Maintain professional standards and follows the treatment policies and procedures of Sea Mar, and of the federal, state, and local regulations. Has a thorough working knowledge of appropriate WAC and RCW's. Maintains accurate documentation on his/her direct services hours and education, to be submitted to the Department of Health, to become a Substance Use Disorder Professional. Design, implement and evaluate individualized treatment plans based on ASAM criteria. Coordinate and cooperate with staff or other agencies and organizations in order to expedite treatment for each patient and for their continuum of care. Complete on time all of the required Federal, State, County, and local forms for each patient (BECCA, CDDA). Attends regular staff meetings and in service training for clinical personnel. Maintains accurate and current records on each patient in accordance to ASAM and according to the facility's procedure for patient records. Prepares current patient progress reports and completion notices and provides them to the courts, probation departments, families and referral sources when appropriate. Performs other related tasks as assigned by the Supervisor. Classification as a Substance Use Disorder Trainee per WAC 246-811. PRODUCTIVITY STANDARD Performs a minimum of 60 hours of face-to-face contact and 60 hours of activities each month. CONDITIONS OF EMPLOYMENT This person must sign an oath of permanent confidentiality covering all patient relate information. This person must receive a background check from DSHS and Washington State Patrol. This person must comply with all ethical standards of Sea Mar and as required by DOH. The person in this position shall have no history or evidence of alcohol or other drug misuse for a period of three (3) years immediately prior to the date of employment at Sea Mar Community Health Centers, and no misuse of alcohol or other drugs while employed at Sea Mar. This person must complete an annual clinical competency test as a Substance Use Disorder Trainee. The Supervisor shall sign and date the job description to affirm the validity of the information herein.
