About The Position

The Stroke Program Manager is responsible for the overall management and coordination of the Stroke Program. This includes ensuring adherence to mandatory requirements and department expectations, practicing with a customer service approach, and providing care as part of an interdisciplinary team. The role involves utilizing resources effectively, providing patient and family education, and documenting care according to guidelines. The manager must also identify and report signs of neglect/abuse, care for terminally ill patients, and maintain necessary licenses and certifications. Addressing age-specific needs and participating in the development, implementation, and evaluation of educational programs for stroke patients and families are key aspects of the position. The role requires timely collection and evaluation of stroke patient charts, tabulation of data for state initiatives, and maintenance of accurate records of patient outcomes, care timeliness, testing, and disposition. Keeping the program updated for pediatric and pregnant stroke patients, communicating with hospital staff regarding protocol deviations, and planning regular meetings with the Stroke Planning Committee and Co-Director are also essential. The manager will communicate issues with the Co-Directors, maintain communication with Brigham & Women’s Hospital for the Teleneurology program, and be responsible for the program's budget. Additionally, the role involves ensuring clinical areas are up-to-date on stroke education, assisting in community education, and performing chart analysis and outcome correlation. The position challenges current practices, identifies process improvement opportunities, and promotes a culture of continuous improvement. This includes assessing clinical and non-clinical outcomes, conducting data collection, analysis, and dissemination, and producing meaningful analyses from external databases and data abstraction tools.

Requirements

  • Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts
  • Baccalaureate Degree in Nursing required
  • Graduate of an accredited School of Nursing; BSN Required.
  • Effective communication skills
  • Excellent presentation and facilitation skills
  • Demonstrated competence in quality data analysis and presentation
  • Minimum of 5 years of experience in Hospital with progressive experience in quality improvement preferred
  • Minimum of 5 years of experience in Quality Database and/or system management preferred

Responsibilities

  • Participates in the development, implementation, and evaluation of educational programs for stroke patients and families.
  • Collects and evaluates all stroke patients’ charts in a timely manner (within one month of admission).
  • Tabulates the forms required by the State DPH Stroke initiative with the stats dept.
  • Keeps accurate records of stroke patients, outcomes; monitors timeliness of care, testing and disposition.
  • Keeps program up to date for pediatric and pregnant stroke patients.
  • Communicates as needed with hospital staff regarding deviations from protocols.
  • Plans a minimum of 4 meetings a year with the Stroke Planning Committee and Co-Director to discuss changes in protocols, policies and procedures, outcomes, in the system.
  • Communicates with the Co-Directors about issues with the Stroke program.
  • Maintains good communication with Brigham & Women’s Hospital in the Teleneurology program.
  • Responsible for the overall budget of the Stroke Program.
  • Keeps the clinical areas where stroke patients are treated, up to date on stroke education.
  • Assists in community education regarding stroke recognition, diagnosis, and treatment.
  • Must be able to work independently.
  • Must be able to perform analysis of stroke patient’s charts in a timely manner.
  • Must be able to get the information to stats dept. in a timely manner.
  • Must be able to correlate outcomes with actual patient care.
  • Performs other related duties as required.
  • Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management.

  • Assesses clinical and non-clinical outcomes using the measurement systems established, including data collection analysis and correlation and dissemination of information to internal customers.
  • Responsible for searching external databases and using data abstraction tools to produce meaningful analyses and correlation of data in simple, understandable graphic format for internal customers.
