About The Position

The STOP Case Manager is responsible for providing casework services to the participants, including intake, assessments, placement in community-based services, case management, and coordination with DAPO. The case manager to participant caseload ratio shall not exceed 1 to 50, unless prior written approval is obtained from Division of Rehabilitative Programs (DRP).

Requirements

  • Possess two (2) years of experience working in a SUDT program providing case management services
  • Minimum of two (2) years of experience working with criminal justice populations in the provision of case management services.

Responsibilities

  • Provide interactive services to the STOP participants.
  • Initiate and develop Case Management Plans (CMP) (aka Action Plan) in ARMS for participants.
  • Develop and monitor the participant’s progress relative to their CMP through all phases of the program.
  • Participant CMP/Action Plans are case conferenced with CBPs weekly to monitor participant progress and completion.
  • Make appropriate referrals to outside agencies as necessary.
  • Maintain progress notes in participant files.
  • Keep the AOR apprised of participant’s progress.
  • Develop discharge and aftercare plans, which include residency accommodations, mental health/ medical information (if applicable), employment, continued education (if applicable), transportation options, and SUD treatment options.
  • Maintain collaborative relationships evidenced by effective communication with STOP Program Director and Director of Operations, CDCR liaisons, subcontracted CBP programs, and STOP participants.
  • Maintain comprehensive case records and accurate statistical information for STOP participants and services in ARMS, including all required elements of the Case Management Plan.
  • Development of CMP within ten (10) calendar days of admission to the CBP modality ensuring goals and tasks are based on assessment(s) results.
  • Provide a minimum of two (2) case management sessions per month with each participant and document in file and in ARMS.
  • Ensure results of evidence-based assessment is documented in Case Management plan, input into ARMS and provided to the Community Based Provider.
  • Performs other related duties as assigned.
