About The Position

The Midnight Staff Pharmacist provides inpatient and decentralized pharmacy coverage while complying with applicable hospital policies, procedures, codes, and standards. The pharmacist may be required to perform the pharmacy director’s essential duties in the director’s absence. The role requires adherence to Memorial Health System’s Standards of Excellence and strict confidentiality.

Responsibilities include supervising pharmacy technicians; supporting medication order entry and prospective drug utilization review; participating in clinical pharmacy services and patient care; and assisting with quality assurance, medication-error reporting, and prevention. The pharmacist also performs other duties as requested by the director of pharmacy.

Requirements

• Current pharmacist licensure in the State of Ohio, or a West Virginia pharmacist license if working in West Virginia.
• Verbal and written communication skills.
• Manual dexterity and near visual acuity.

• Some experience in a hospital pharmacy setting is preferred.

Responsibilities

• Provide coverage for the central inpatient pharmacy and decentralized pharmacy service areas as requested.
• Participate in inventory and stock control.
• Oversee and supervise pharmacy technicians, providing direction and training as necessary.
• Provide drug information to hospital professional staff and patients as necessary.
• Assist with preparation of sterile products, chemotherapy, and compounded products.
• Oversee or assist with Pyxis restocking and troubleshooting.
• Interpret and clarify physician orders, and supervise their filling in accordance with pharmacy laws, accepted practices, available resources, and clinical judgment.
• Perform prospective drug utilization evaluation with every order entry.
• Participate in medication profile reviews, drug therapy and pharmacokinetic consultations and dosing, renal dosing adjustments, parenteral nutrition dosing, inpatient multidisciplinary rounds, and patient education as needed.
• Monitor drug therapy, relevant laboratory information, and drug levels, and provide interventions when required.
• Assist with antibiotic selection based on culture and sensitivity results.
• Monitor and review laboratory values for patients receiving parenteral nutrition.
• Assist with and recommend IV-to-oral conversions when necessary.
• Respond to code blue situations and assist with medication interventions.
• Assist in coordinating and implementing quality assurance activities.
• Assist with reporting and prevention of medication errors and adverse drug events.
• Perform other duties and responsibilities as requested by the director of pharmacy.
• In the pharmacy director’s absence, perform the director’s essential duties as required.
• Comply with hospital policies, procedures, codes, and standards, and maintain strict confidentiality.
