Offering Independent Living, Assisted Living, as well as a 40-bed Skilled Nursing Unit providing Long-term Care and Short-term, sub-acute rehab services. Day shift 7:00am to 7:00pm, schedule will include every other weekend Position is based on the Assisted Living floor Pay range: $35 - $40 per hour based on experience Benefits may include depending on full or part-time status: Access to health, dental and vision benefits 401k retirement plan with company match Education Assistance Program Company paid life insurance Access to other benefits like Short Term Disability and Accident coverage Generous paid time off Paid Holidays About the Position: Staff Nurse provides compassionate care to patients, residents, and clients; Assesses, plans, coordinates, and implements effective care for Assisted Living residents; Administers medications, treatments, updates documentation and responds to emergency calls in an accurate and timely manner; Provides guidance to non-licensed care staff to help ensure continuity of care. Assists with administrative duties as requested.

Requirements

  • High School diploma or equivalent
  • Currently licensed as an RN/LPN in the state of Illinois
  • Ability to communicate with residents and staff
  • Basic computer knowledge and key boarding skills; ability to learn company systems
  • Ability to work holiday and weekend hours as well as to be on call
  • Approachable, customer service orientation
  • Strong interpersonal skills
  • Think and act calmly and logically to meet unusual occurrences of the job without being thrown off stride
  • Detail orientation
  • Must have ability to perform duties accurately and rapidly under tight deadlines with minimal supervision
  • Must be able to take direction and fulfill task requirements
  • Demonstrate competencies in operating emergency equipment
  • Demonstrated ability to administer medications and treatments in a timely manner in accordance with facility policy

Responsibilities

  • Administer medications and treatments, including, but not limited to: dressings, catheterization, suctioning, oxygen, etc.
  • Maintain accurate documentation of medication administered and care rendered
  • Assist in admission and discharge procedures
  • Respond to resident emergency call system as indicated
  • Make rounds with physicians and transcribes orders
  • Keep physician and family aware of change in resident's condition
  • Maintain the self-respect, personal dignity, and physical safety of each resident
  • Assure continuity of services and care
  • Understand, participate and assist in implementing the Quality Improvement Program
  • Utilize the Quality Improvement Program for identification, placement, and monitoring of residents in the appropriate programs
  • Notify other departments of resident changes (room changes, LOA, new admits, discharges, etc.)
  • Conduct resident and family education
  • Document according to established procedures for charting, i.e., medication, treatments, progress notes, etc.
  • Report any discrepancies noted concerning physician's orders, diet changes, charting errors, etc. to supervisor
  • Transcribe physician's orders to resident charts, cardex, medication/treatment cards and care plans
  • Chart progress notes in an informative, factual manner that reflects the care administered as well as the resident's response to care
  • Accurately identify skin changes and implement/follow facility skin management protocols
  • Supervise and assist with meal service
  • Consult with dietary regarding the dietary needs of the residents and follow through on dietary recommendations
  • Monitor resident with significant weight loss and gain and report to the physician and dietary department
  • Identify residents at risk for nutritional problems and implement preventative measures
  • Check items of food that are brought in by family members to assure they comply with resident orders, dietary needs, and safety
  • Assist in completion of the care plans and is involved in the care conference as deemed necessary
  • Assist in the completion of the MDS and demonstrate knowledge of the MOS process
  • Ensure that the progress notes are reflective of the care plan and approach is being followed
  • Assure that assigned Certified Nursing Assistants (CNAs) are aware of the resident care plans, and ensure that the nursing assistant is familiar with plan of care
  • Adhere to the policies and procedures of the business with the highest practical standards
  • Participate in and maintain current status on all training and education required by the company
  • Communicate effectively with all members of the staff and business partners
  • Provide feedback to others for continuous quality improvement of the entire organization
  • Openly accept feedback from others for the continuous quality improvement of the entire organization
  • Escalate issues when there is appearance that business standards of quality are not present
  • Comply with all rules and regulations of the organization, including HIPAA and safety
  • Perform other duties as assigned
