Provides case coordination and delivers community rehabilitation services. The role involves conducting home visits, obtaining psychosocial histories, conducting functional skills assessments, and establishing liaison with psychiatric care providers. The position ensures client access to psychiatric, substance use, medical services, and entitlement benefits, while also monitoring client medication adherence and side-effects. Responsibilities include administering health screening tools, developing individualized rehabilitation plans (Service/Support Plans) in consultation with the treatment team, and delivering various community rehabilitation services such as Assertiveness/Self Advocacy Training, Community Integration Services/Resource Development, Daily Living Skills Training, Health Services, Medication Management Training, Parenting Training, Rehabilitation Counseling, Skill Development Services, Socialization, Substance Abuse Services, and Symptom Management. Additionally, the role provides housing support services in areas like education/employment support, behavioral health, harm reduction/substance use, assistance with entitlements, healthcare, emergency services, Health Home Care Coordination, and transportation. The position requires documenting client progress, training clients in safety procedures, preparing residents for transition to independent living, facilitating crisis intervention, staffing the Emergency Phone, maintaining client charts and submitting reports, overseeing apartment maintenance, attending in-service training, providing coverage for other staff, serving as a "Qualified Mental Health Staff Person", and performing other duties as assigned by the supervisor.
