This position provides case coordination and delivers community rehabilitation services for clients in single and scattered site settings. The role involves conducting home visits, obtaining psychosocial histories, assessing functional skills, and liaising with psychiatric care providers to ensure access to necessary services. The Case Manager will develop and implement individualized rehabilitation plans, deliver various community rehabilitation services, monitor client progress, and document responses to treatment. Responsibilities also include training clients in safety procedures, preparing them for independent living, facilitating crisis intervention, and maintaining client charts and apartment upkeep. The role requires serving as a Qualified Mental Health Staff Person and performing other duties as assigned by the supervisor.
