Provides case coordination and delivers community rehabilitation services. The role involves conducting home visits, obtaining psychosocial histories, conducting functional skills assessments, and liaising with psychiatric care providers. The position ensures client access to psychiatric, substance use, medical services, and entitlement benefits, while also monitoring client medication and administering health screenings. A key responsibility is developing and implementing individualized rehabilitation plans (Service/Support Plans) in collaboration with the treatment team. The role also includes delivering various community rehabilitation services such as Assertiveness/Self Advocacy Training, Community Integration Services, Daily Living Skills Training, Health Services, Medication Management Training, Parenting Training, Rehabilitation Counseling, Skill Development Services, Socialization, Substance Abuse Services, and Symptom Management. Additionally, housing support services are provided in areas like education/employment support, behavioral health, harm reduction, entitlement assistance, healthcare, emergency services, Health Home Care Coordination, and transportation. The position requires documenting client progress, training clients in safety procedures, preparing residents for transition to independent living, facilitating crisis intervention, and maintaining client charts and reports. The role may also involve staffing the Emergency Phone, overseeing apartment maintenance, attending in-service training, and providing coverage for other team members. The position serves as a "Qualified Mental Health Staff Person" and performs other duties as assigned by the supervisor.
