About The Position

Anna's House, part of Catholic Charities of Baltimore, is seeking a Case Manager to provide intensive case management services, including as-needed home visitation. The role involves assessing household needs, coordinating resources, and offering advocacy, information, and referral services. Anna's House offers eight onsite transitional living spaces and a range of homeless services, including emergency shelter, transitional housing for families fleeing domestic violence, community supportive housing, and rapid re-housing. The organization aims to provide personalized assistance and encouragement to help clients achieve self-sufficiency and recognize their capabilities.

Requirements

  • Bachelor's degree in Human Services, Social Work, or related field.
  • Two years of related experience.
  • Excellent knowledge of case management and crisis intervention services.
  • Proficiency in using computer systems and software, including basic Windows PC, web browsing, and Microsoft Outlook.
  • Ability to remain in a stationary position, which may involve standing or sitting for extended periods.
  • Ability to move about to accomplish tasks or transition between different work sites.
  • Ability to engage in communication with others to exchange information.
  • Ability to perform repetitive motions involving the wrists, hands, and/or fingers.
  • Ability to assess the accuracy, neatness, and completeness of assigned work.
  • Hearing and vision abilities, with or without correction, sufficient to drive safely and effectively observe and communicate with others.

Nice To Haves

  • Knowledge of other Microsoft Office applications, such as Word, Excel, PowerPoint, Teams, and OneDrive is preferred.

Responsibilities

  • Develops Individualized Service Plans (ISPs) in collaboration with participants, focusing on self-sufficiency and tracking progress.
  • Conducts self-sufficiency assessments using the Arizona Self-Sufficiency Matrix at intake and closure, documenting and communicating results.
  • Maintains consistent contact and conducts required visits to evaluate progress, address challenges, and ensure participant engagement.
  • Advocates effectively for individuals, families, and children, securing needed community services and following up.
  • Collaborates with staff and volunteers to engage clients in programs supporting self-sufficiency.
  • Maintains accurate and up-to-date case files and enters data into the Service Point database.
  • Assists clients in accessing and completing applications for eligible benefits (e.g., emergency cash assistance, SNAP, day care vouchers, eviction prevention).
  • Collects, inputs, and reports data according to program requirements, keeping supervisors informed of trends and outcomes.
  • Utilizes Motivational Interviewing and strength-based coaching to help clients overcome barriers.
  • Fosters an environment aligned with Trauma Informed Care (TIC) principles.
  • Participates in required meetings, trainings, and program events.
  • Maintains a working knowledge of program services and connects clients to internal resources.
  • Performs other duties as assigned.
  • Participates in local Continuum of Care Meetings and Committees (Anna's House specific).
  • Facilitates Life Skills classes and provides aftercare services for up to twelve months (Anna's House specific).
  • Provides direction to Program Assistants in support of case management (Anna's House specific).
  • Ensures compliance with HUD rules and regulations and passes audits (Housing Services specific).
  • Coordinates housing leased to or for clients, negotiates leases, and maintains contact with realty representatives (Housing Services specific).
  • Inspects property and/or apartments at specific intervals (Housing Services specific).
  • Reinforces occupancy policies and procedures by initiating procedures for non-payment of rent (Housing Services specific).
  • Monitors the upkeep of the property, including maintenance and repairs (Housing Services specific).
