About The Position

Anna's House, a program of Catholic Charities of Baltimore, is seeking a Case Manager to provide intensive case management services, including as-needed home visitations, to participants. The role involves assessing household needs, coordinating resources, and offering advocacy, information, and referral services. Anna's House offers transitional living spaces and a range of homeless services, aiming to empower individuals and families towards self-sufficiency through personalized assistance and encouragement.

Requirements

  • Bachelor’s degree in Human Services, Social Work, or related field.
  • Two years related experience.
  • Excellent knowledge of case management and crisis intervention services.
  • Demonstrates patience and composure when working with others.
  • Exhibits exceptional relational and customer service skills.
  • Acts with discretion, tact, and professionalism.
  • Demonstrates excellent verbal and written communication skills.
  • Displays excellent organizational skills and attention to detail.
  • Utilizes effective conflict resolution skills.
  • Proficiency in using computer systems and software, including basic Windows PC, web browsing, and Microsoft Outlook.

Responsibilities

  • Develops Individualized Service Plans (ISPs) in collaboration with participants, focusing on self-sufficiency goals and tracking progress.
  • Conducts self-sufficiency assessments at intake and closure using the Arizona Self-Sufficiency Matrix and documents results.
  • Maintains consistent contact and conducts required visits to evaluate progress, address challenges, and ensure participant engagement.
  • Advocates for individuals, families, and children to secure needed community services and follows up to confirm support.
  • Collaborates with staff and volunteers to engage clients in programs supporting self-sufficiency and ensures service alignment with goals.
  • Maintains accurate case files and enters data into the Service Point database.
  • Assists clients in completing applications for benefits like emergency cash assistance, SNAP, and day care vouchers.
  • Collects, inputs, and reports data according to program requirements and informs supervisors of trends and outcomes.
  • Utilizes Motivational Interviewing and strength-based coaching to help clients overcome barriers.
  • Fosters an environment aligned with Trauma Informed Care (TIC) principles.
  • Participates in required meetings, trainings, and program events.
  • Maintains knowledge of program services and connects clients to internal resources.
  • Performs other duties as assigned.
  • Participates in local Continuum of Care Meetings and Committees.
  • Facilitates Life Skills classes and provides aftercare services for up to twelve months.
  • Provides direction to Program Assistants in support of case management.
  • Ensures compliance with U.S. Department of Housing and Urban Development (HUD) rules and regulations.
  • Coordinates housing leased to or for clients, negotiates leases, and maintains contact with realty representatives.
  • Inspects property and/or apartments at specific intervals.
  • Reinforces occupancy policies and procedures.
  • Monitors the upkeep of the property, including snow removal, trash removal, landscaping, and routine maintenance.
