Requirements

• Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting.
• A compassionate and patient-centered approach.
• Ability to support patients and families from diverse backgrounds and belief systems.
• Strong communication and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Valid driver’s license and reliable transportation for travel within the service area.

Responsibilities

• Provide spiritual support and counseling to patients, families, and caregivers.
• Assess spiritual needs and help incorporate them into the patient’s overall plan of care.
• Offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care.
• Collaborate with nurses, social workers, aides, and other members of the interdisciplinary team.
• Participate in care planning and team meetings.
