About The Position

The Hospice Spiritual Coordinator provides spiritual care to hospice patients and their families. The role conducts initial and ongoing spiritual assessments, develops individualized care plans, and collaborates with the interdisciplinary team to integrate spiritual needs into overall patient care. The coordinator offers spiritual guidance, counseling, and emotional support while respecting patients’ cultural and religious beliefs, and supports end-of-life concerns and bereavement. The position also includes educating patients, families, and staff about spiritual care and maintaining accurate, timely service documentation.

Requirements

• Bachelor’s or Master’s degree in divinity, Theology, Pastoral Care, or a related field.
• Two years of experience working in a hospice, counseling, or ministry environment.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Conduct initial and ongoing spiritual assessments to identify hospice patients’ and families’ needs, beliefs, and preferences.
• Develop individualized spiritual care plans and collaborate with the interdisciplinary team to incorporate spiritual needs into overall patient care.
• Provide spiritual guidance, counseling, and emotional support while respecting cultural and religious beliefs.
• Support patients and families with end-of-life concerns, including meaning, faith, forgiveness, and personal values.
• Provide bereavement support to families before and after a patient’s passing.
• Educate patients, families, and staff on spiritual care.
• Maintain accurate, timely documentation of services provided.
