About The Position

FeldCare Connects is seeking a self-motivated Speech Therapist to provide home health care in the Santa Fe Springs area, working with adults of all ages. The role assesses and evaluates patients' care needs related to speech and language disorders, communication functionality, swallowing, and cognitive impairments. The Speech Therapist develops and carries out treatment plans and interventions to improve patients' well-being and monitors their progress.

The position is for an independent contractor. Clinicians can make their own schedules and work in their preferred area, with administrative support for assignment, communication, scheduling, care coordination, and quality assurance.

Requirements

• Speech Therapist license and registration by the state.
• Completion of an accredited Speech Therapist program.
• Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech-Language-Hearing Association.
• Ability and enthusiasm to work with people of all ages.

Responsibilities

• Interpret, plan, establish, and execute an appropriate treatment program in accordance with the physician's referral.
• Observe, record, and report patients' responses to treatment and changes in their conditions.
• Instruct and monitor patients, their families, and other health team personnel regarding speech therapy procedures related to the patient's program.
• Make recommendations based on the home setting and the individual needs of the patient.
• Record each patient visit and add detailed notes to the medical record.
• Review patients' responses to treatment and make recommendations to the physician about whether speech therapy should continue or be discontinued.
