About The Position

FeldCare Connects is seeking a self-motivated Speech Therapist to provide home health care to adults of all ages in the Yucca Valley area. The therapist assesses and evaluates patients’ needs related to speech and language disorders, communication functionality, swallowing, and cognitive impairments. Based on those evaluations, the therapist develops and carries out treatment plans, delivers interventions to improve patients’ well-being, and monitors their progress.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Speech Therapist license and registration by the state.
• Completion of an accredited Speech Therapist program.
• Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech-Language-Hearing Association.

Responsibilities

• Interpret, plan, establish, and carry out an appropriate treatment program in accordance with the physician’s referral.
• Observe, document, and report patients’ responses to treatment and changes in their condition.
• Instruct and monitor patients, families, and other health team personnel on speech therapy procedures related to the patient’s program.
• Make recommendations based on the home setting and each patient’s individual needs.
• Record each patient visit and enter detailed notes into the medical record.
• Review responses to treatment and recommend to the physician whether speech therapy should continue or be discontinued.
