About The Position

FeldCare Connects is seeking a self-motivated Speech Therapist to provide home health care to adults of all ages in the Del Mar area. The role assesses and evaluates patient care needs related to speech and language disorders, communication functionality, swallowing, and cognitive impairments. The therapist develops and carries out treatment plans and interventions to improve patients’ well-being and monitors their progress.

The Speech Therapist works in accordance with physician referrals, documents each visit in the medical record, and communicates with patients, families, physicians, and other health team personnel about treatment, patient responses, and changes in condition. The position is an independent contractor opportunity. Clinicians can set their own schedules and preferred service area, with administrative support for assignments, communication, scheduling, care coordination, and quality assurance.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Speech Therapist license and state registration.
• Completion of an accredited Speech Therapist program.
• Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech-Language-Hearing Association.

Responsibilities

• Interpret, plan, establish, and execute an appropriate treatment program in accordance with the physician’s referral.
• Review, record, and report the patient’s response to treatment and any changes in the patient’s condition.
• Instruct and monitor patients, their families, and other health team personnel regarding speech therapy procedures related to the patient’s therapy program.
• Make recommendations based on the home setting and the individual needs of the patient.
• Record each patient visit and enter detailed notes into the medical record.
• Review patient responses to treatment and recommend to the physician whether speech therapy should continue or be discontinued.
