About The Position

Pinnacle Home Care is hiring a PRN Speech Therapist (ST/SLP) to provide compassionate, patient-centered care in clients’ homes. The role involves evaluating and treating speech, language, and hearing abilities; coordinating care with physicians and interdisciplinary team members; educating clients and families; and documenting assessments, treatments, and progress. Pinnacle Home Care is Florida’s largest Medicare-certified home health provider and has delivered patient-centered care for over two decades.

Requirements

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• At least two years of clinical experience as a Speech Therapist.
• Current BLS Certification.
• Valid driver’s license and insured, reliable vehicle.
• Strong emotional resilience and physical stamina, including the ability to stand, stoop, and bend frequently.
• Excellent judgment, communication, and observational skills.
• Self-motivated, with the ability to work independently.

Responsibilities

• Evaluate clients’ speech, language, and hearing abilities and recommend strategies to improve communication and hearing.
• Conduct initial and ongoing comprehensive assessments, including OASIS evaluations.
• Develop and revise treatment plans in consultation with physicians as needed.
• Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
• Educate clients and families on speech therapy techniques and related challenges.
• Promptly document evaluation data, treatments, and client progress.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
• Communicate regularly with physicians, confirm orders are appropriate, and address changes in clients’ conditions.
• Participate in periodic care-plan revisions, update physicians on client status, and obtain additional orders as required.
