About The Position

Avondale Health and Rehab is hiring a Speech Therapist SLP to plan, organize, develop, and direct Speech Language Pathology services in accordance with applicable federal, state, and local standards, guidelines, and regulations, as well as direction from the Administrator. The goal is to maintain the highest degree of quality resident care. The position serves residents at Avondale Health and Rehab and Dyersburg Health and Rehab, requiring travel between the two locations. The role may be full-time with flexible scheduling or per diem/PRN. Pay is based on years of experience, and benefits may vary based on employee status.

Requirements

Master's degree in Speech Language Pathology.
Certificate of Clinical Competency.
Active SLP license in the state or states of practice.
Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

Provide quality speech therapy services and initiate referrals when appropriate, following established standards of practice, therapy department procedures, and productivity standards.
Evaluate residents and obtain data necessary for treatment planning and implementation.
Develop treatment plans with target lists and short-term and long-term goals.
Instruct patients, families, and staff in appropriate speech-language procedures to support carryover of therapeutic remediation and goals in daily activities.
Complete compliant, medically necessary documentation consistent with CMS expectations using NetHealth EMR systems.
Oversee and implement proper FEES/MBS protocols in coordination with site programs.
Participate in screening protocols to ensure residents' needs are met promptly and with clinical excellence.
