About The Position

River Falls Post Acute, located in Marietta, South Carolina, is a 44-bed facility offering a low-key, fun work environment where employees are valued, appreciated, and supported. The close-knit team is dedicated to providing exceptional care while helping residents achieve their highest level of independence and quality of life. This position offers flexible scheduling, reliable hours, and a supportive therapy team. We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. In this role, you will evaluate and treat residents with communication, cognitive, and swallowing disorders while collaborating with physicians, nursing staff, families, and other healthcare professionals to improve resident outcomes and quality of life.

Requirements

  • Master's or Doctoral degree in Speech-Language Pathology required
  • Licensed as a Speech-Language Pathologist in the state in which services are provided
  • Current Speech-Language Pathologist license in good standing
  • Must maintain all required continuing education requirements
  • Must remain in good standing with the applicable State Board
  • Strong clinical, documentation, and communication skills

Nice To Haves

  • Experience in skilled nursing, rehabilitation, or long-term care preferred
  • Experience with PointClickCare (PCC) and Casamba preferred
  • Passion for helping residents improve communication, cognition, and swallowing function

Responsibilities

  • Evaluate residents and develop individualized treatment plans
  • Provide speech, language, cognitive, and swallowing therapy services in accordance with physician orders
  • Monitor resident progress and modify treatment plans as appropriate
  • Communicate with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomes
  • Complete accurate and timely documentation in compliance with facility, regulatory, and payer requirements
  • Participate in care planning meetings and discharge planning activities
  • Educate residents, families, and caregivers on treatment plans and maintenance programs
  • Maintain compliance with all state practice acts, facility policies, and regulatory requirements
  • Promote resident safety, independence, and quality of life
