Requirements

Nice To Haves

Experience providing therapy to adults with speech, language, cognition, and swallowing disorders in acute care or skilled nursing settings preferred.

Responsibilities

Assess and diagnose speech, language, cognitive, and swallowing disorders for adult patients in an acute care / skilled nursing setting.
Develop and implement individualized therapy plans with measurable goals; adjust plans based on patient progress.
Provide direct therapy sessions (individual and, when appropriate, group) aligned to patient goals.
Perform bedside swallow evaluations and manage dysphagia therapy in collaboration with the interdisciplinary team.
Document evaluation findings, treatment sessions, progress notes, and discharge planning in the electronic medical record; communicate progress with physicians and the care team.
Partner with rehabilitation, nursing, dietary, and social work teams to support discharge planning and caregiver education.
