About The Position

Adaptive Home Health is building a higher-acuity, patient-centered skilled home health model across Texas to improve patient access to home health care. The Speech-Language Pathologist is a core part of that mission, helping patients restore communication, cognition, and swallow safety so they can function safely at home, while building caregiver carryover so progress continues between visits. This field-based role is responsible for the patient’s SLP plan, progression, and functional outcomes. The clinician completes in-home evaluations, delivers skilled visits, assesses and treats communication and cognitive-communication disorders, and addresses dysphagia and swallow safety within home health scope and orders. The SLP develops measurable, functional plans of care, coordinates with the multidisciplinary team, coaches patients and caregivers, and documents promptly and thoroughly in Homecare Homebase (HCHB). The role is full-time, Monday through Friday, with occasional weekends as needed. Compensation is $75 per productivity point. Adaptive describes support from experienced clinical and operational teams, schedule flexibility and smart routing, and clinician-focused charting and scheduling technology.
