About The Position
A full-time Speech Language Pathologist (SLP-CCC) is sought for a Skilled Nursing Facility in Sunnyvale, California. The role is a direct-hire, permanent placement on a collaborative rehabilitation team that provides high-quality rehabilitation services. Requirements
- Graduate of an accredited Speech-Language Pathology program.
- Current California SLP license or eligibility for licensure.
- CCC-SLP required for this location.
- Strong interpersonal and documentation skills.
Responsibilities
- Evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders.
- Develop and implement individualized treatment plans.
- Document patient progress using EMR systems.
- Collaborate with physical therapists, occupational therapists, nurses, and physicians to deliver coordinated care.
- Educate patients, families, and staff on treatment goals and progress.
