About The Position

Under the direct supervision of the Director of Specialty Services Operations, and the clinical oversight of the Director of Ambulatory Care, the Specialty Ambulatory Care Manager, RN operates as a key clinical leader supporting Specialty Care. This role collaborates regularly with the Associate Chief Medical Officer of Specialty Services, working effectively to strengthen the partnership between clinical and operations leadership and ensuring the implementation of the San Ysidro Health model of care which is designed to deliver safe high-quality care and improve the health outcomes of the communities we serve. The Specialty Ambulatory Care Manager, RN hires, trains, and manages all assigned Specialty Care nursing staff; ensuring appropriate training and resource support are provided for the delivery of licensed and non-licensed clinical care at assigned clinics. This role identifies the population specific opportunities to increase health potential, improve health outcomes, and close care gaps by monitoring provider, care team, and clinic performance data. This role also serves as the Clinical Quality Improvement lead for the Specialty department overseeing applicable quality improvement initiatives at assigned clinics.

Requirements

  • Five years of clinical Registered Nursing experience with direct reports and/or unit oversight.
  • Demonstrated ability to manage and lead quality improvement projects.
  • Demonstrated ability to be culturally sensitive and appropriate in working with others.
  • Demonstrated respect for diversity.
  • Graduate of an accredited registered nursing program
  • Associate Degree of Nursing (ADN)
  • Current California Registered Nurse (RN)
  • BLS Certification
  • Excellent written and verbal communication skills.
  • Flexible, organized, motivated, multi-tasker and excellent time management skills.
  • Strong interpersonal skills and ability to work efficiently and effectively with medical providers and nursing staff.
  • Comfortable with computer-based tools, processes and workflows.

Responsibilities

  • Ensures assigned nursing staff provide the highest quality care while working within the parameters of their respective scope (s). All work to be performed in compliance with the Nurse Practice Act, the medical assistant scope of practice, the National Committee for Quality Assurance, the Joint Commission, as well as applicable federal, state, and local requirements.
  • Facilitates coordinated care efforts that align with our patient-centered model to help achieve SYHealth’s patient safety, satisfaction, and clinical quality goals.
  • Works in collaboration with all members of the care team to support the healthcare needs of assigned populations through continuous monitoring of data, trends identification, care team development, and implementation of workflow efficiencies, pilots, and initiatives.
  • Actively participates in the development, implementation, and monitoring of departmental protocols, policies, and procedures that support achievement of organizational goals and objectives.
  • Supports the development of service line staffing standards, assisting in the planning and identification of specific roles and responsibilities of the Care Team.
  • Selects, trains, supervises and develops Care Teams members capable of meeting the clinical support needs of the assigned service lines and communities served.
  • Is responsible for Specialty Services in the annual coordination of required continuing education schedules for assigned staff, service lines, and sites.
  • Ensures the clinical competency of assigned nurses and medical assistants prior to assignment of independent care activities. Validates the clinical competency of nurses and medical assistants annually and as needed.
  • Establish consistent communication mechanisms and regular meetings with the Director of Specialty Services Operations, the Director of Nursing, and assigned care teams to review and discuss the state of operations and develop actions and plans for improving outcomes.
  • Ensures assigned care teams provide timely patient triage, new patient intakes, follow-up visits, and provider co-visits to ensure effective clinical workflow and maximize patient access to care.
  • Collaborates with medical providers to ensure proper execution of provider orders and patient care plans.
  • Monitors all assigned Specialty clinic pools and in-basket tasks/messages to ensure prompt handling and disposition in accordance with SYHealth standards.
  • Ensures that appropriate information is entered into the electronic health record. Conducts intermittent chart reviews to evaluate nursing and medical assistant documentation; identifies areas for improvement and/or staff training needs.
  • Responsible for implementation of team-based care, including the facilitation of care team huddles and Practice Transformation Meetings.
  • Conducts clinical quality improvement and evaluation activities and guides the implementation of quality improvement projects in the clinic setting.
  • Responsible for the preparation of monthly narrative and/or statistical reports as required federal/state/local/organizational reporting mandates.
  • Follows safety procedures and reports unsafe conditions.
  • Represents SYHealth at various meetings and conferences.
  • Participates in Ambulatory Care meetings as requested by the Director of Nursing
  • Participates in all multi-disciplinary meetings, daily huddles, SYHealth staff meetings, and selected staff development trainings, as directed.
  • Maintains a flexible schedule, allowing for some weekend work as well as emergency coverage to help meet clinic needs and related activities/events.
  • Adapts to shifting priorities, demands and timelines through analytical and problem-solving capabilities.
  • Travels to multiple sites within the SYHealth network.
  • Performs other related activities assigned.
