In Person at 3-4 skilled nursing facilities in the Long Beach area a week Four 10hr shifts, 8-6:30 or five 8hr shifts M-F You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Requirements

Undergraduate degree or equivalent experience
BSW - 1+ years of clinical experience in a healthcare setting, or successful completion of a relevant internship
Driver’s License and access to reliable transportation

Nice To Haves

Bachelor's degree from a CSWE-accredited School of Social Work or a bachelor’s degree in a Behavioral Science-related field

Responsibilities

Utilize vetted and approved resources to establish a safe, effective care management plan
Complete a comprehensive psychosocial assessment for each member and develop a member-specific action plan
Collaborate with members, families, and healthcare providers
Identify and initiate referrals to financial, psychosocial, community, and state supportive services
Advocate for members and assist them in navigating the healthcare system
Address age-specific communication needs and engage effectively with members and families from diverse backgrounds
Identify and intervene with high-risk members requiring social services
Maintain records of social care treatment plans, interventions, and outcomes
Participate in interdisciplinary team meetings
Act as a resource to department teammates, organization teammates, and providers for complex members
Understands insurance products, benefits, coverage limitations, insurance, and governing regulations as it applies to the health plan
Use, protect, and disclose members’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
Portray a positive attitude and professionalism when interacting with others
Trained to assess and respond to danger to self or others in alignment with state licensure standards
Perform additional duties as assigned
