About The Position

Plans, coordinates, and implements social services for patients of all ages, their families, and other people significant to patients at the Medical Center. Anticipates and addresses patients’ psychosocial, financial, equipment, and educational needs, as well as their need for community resources, from admission through transfer to the next appropriate level of care. Assists with discharge planning, grief counseling, financial applications, and access to community resources as needed. The role enhances patient care and contributes to the goals and mission of the Medical Center.

Requirements

• Master’s degree in social work from an accredited social work (MSW) program.
• Current state social work licensure.
• For Duluth Hospice: valid motor vehicle operator’s license, verified through the appropriate licensing agency.
• For Duluth Hospice: proof of current motor vehicle insurance.
