About The Position

Rutherford Regional Health System is seeking a full-time Licensed Master's Social Worker (LCSW) to support patients and their families, caregivers, and legal representatives. The role involves assessing needs, coordinating services and interventions, participating in case reviews, collaborating with healthcare team members to support treatment-plan compliance, and assisting with discharge planning. The social worker identifies resources such as transportation, housing, healthcare, and social or spiritual services, and provides referrals as part of discharge planning. The position also performs Medicaid screenings for NHP and helps patients complete Safety Net applications when needed. The schedule is 8:00 a.m. to 4:30 p.m.

Requirements

• Master’s degree.
• State of Michigan Licensed Master's Social Worker (LCSW).
• Basic Life Support Health Care Provider (BLS-HCP) certification.
• Crisis Intervention Training within 90 days of hire or transfer.
• Two years of related work experience.

Responsibilities

• Interview and assess patients and/or their family members, caregivers, or legal representatives.
• Determine, prioritize, provide, and/or arrange needed internal and external services or interventions.
• Participate in case reviews to evaluate case management and patient progress.
• Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
• Assist with discharge planning and processes.
• Identify appropriate resources, including transportation, housing, healthcare, and social or spiritual services, and provide referrals as part of the discharge plan.
• Perform Medicaid screenings as indicated for NHP.
• Assist patients with Safety Net applications when needed.
