About The Position

Rutherford Regional Health System, part of Lifepoint Health, is hiring a Social Worker (LCSW) to support patients and their families, caregivers, and legal representatives. The role focuses on assessing patient needs, arranging appropriate services, coordinating care with healthcare team members, and supporting discharge planning. The schedule is full-time, 8:00 a.m. to 4:30 p.m.

Requirements

• Master’s degree.
• State of Michigan Licensed Master's Social Worker (LCSW).
• Basic Life Support Health Care Provider (BLS-HCP) certification.
• Crisis Intervention Training within 90 days of hire or transfer.
• At least 2 years of related experience.

Responsibilities

• Interview and assess patients and/or their family members, caregivers, or legal representatives.
• Determine, prioritize, provide, and/or arrange needed internal and external services and interventions.
• Participate in case reviews to evaluate case management and progress.
• Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
• Assist with discharge planning and processes.
• Identify appropriate resources, including transportation, housing, healthcare, and social or spiritual services, and provide referrals as part of the discharge plan.
• Perform Medicaid screenings as indicated for NHP.
• Assist patients with Safety Net applications when needed.
