About The Position
Plans, coordinates, and implements social services for patients of all ages, their families, and other significant people in patients’ lives at the Medical Center. Anticipates and addresses patients’ psychosocial, financial, equipment, and educational needs, as well as connections to community resources, from admission through transfer to the next appropriate level of care. Assists with discharge planning, grief counseling, financial applications, and access to community resources as needed. Enhances patient care and contributes to the goals and mission of the Medical Center.
Requirements
- Master’s degree in social work from an accredited social work program.
- Current state social work licensure.
- For Duluth Hospice: valid motor vehicle operator’s license, as verified through the appropriate licensing agency.
- For Duluth Hospice: proof of current motor vehicle insurance.
Responsibilities
