About The Position

Agape Care Group is seeking a hospice medical social worker to support patients and families during end-of-life care. The Social Worker conducts psychosocial evaluations and provides ongoing counseling and emotional support in accordance with each patient’s plan of care. The role contributes to compassionate, meaningful experiences for patients facing life-limiting illnesses and their families. The position offers access to a supportive leadership team and professional development opportunities, with room for advancement. Pay is determined by years of experience and location.

Requirements

A heart to serve patients and families and a passion for providing the best possible care.
Master of Social Work degree from an accredited school of social work approved by the Council on Social Work Education.
Current state license as a social worker.
At least 2 years of clinical work experience.
Reliable transportation.
Ability to sit, stand, bend, and move intermittently, and to lift at least 25 pounds and effectively bear the weight of an average adult.

Nice To Haves

Clinical work experience in healthcare or hospice.
