About The Position
We are seeking a dedicated Social Worker to join a team in an acute care setting. The role supports patients and their families as they navigate complex medical and emotional challenges. This is a day-shift position with 40 hours per week and 8-hour workdays.
Requirements
- A Master’s Degree in Social Work from an accredited program is required in California and Hawaii.
Nice To Haves
- One year of experience in an acute or long-term medical setting is preferred.
- Two years of experience in an acute or long-term medical setting is preferred.
Responsibilities
- Provide assessment, planning, and intervention for patients and families.
- Collaborate with healthcare teams to create comprehensive care plans.
- Assist patients in accessing resources and support services.
- Conduct individual and group counseling sessions as needed.
- Document services and maintain accurate records in compliance with standards.
