About The Position

Crescent Hospice is seeking a patient-centered Licensed Medical Social Worker (MSW) to provide social work services to hospice patients and their families. The role provides counseling and guidance to support emotional well-being and coping capacity, and helps patients and loved ones navigate challenging times with dignity, comfort, and compassion.

The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. The coverage area includes Beaufort, Jasper, Hampton, and Colleton County.

Requirements

• Master of Social Work degree from an accredited school of social work.
• Licensed as a Social Worker by the State Board of Social Work.
• Current driver’s license and vehicle insurance, and access to a dependable vehicle.
• At least one year of experience as a social worker in a healthcare setting.

Nice To Haves

• At least one year of experience as a social worker in a hospice setting.
• Experience in a long-term care or medical-surgical/acute care setting.

Responsibilities

• Assess patients’ psychosocial status and communicate findings to the RN Case Manager and interdisciplinary group.
• Evaluate patients’ medical needs, home situations, financial resources, and community support.
• Implement individualized social work plans and collaborate with the Bereavement Coordinator.
• Contribute to care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
