About The Position

Agape Care Group is seeking a part-time hospice medical social worker to provide psychosocial evaluations and ongoing counseling to patients and families during end-of-life care. Working in accordance with each patient’s plan of care, the social worker provides emotional support to patients and families. The role offers access to supportive leadership and professional development opportunities, with room for advancement.

Requirements

• Master of Social Work degree from an accredited school of social work approved by the Council of Social Work Education.
• Current state social worker license.
• At least 2 years of clinical work experience; healthcare or hospice experience is preferred.
• Reliable transportation.
• Ability to sit, stand, bend, and move intermittently; lift at least 25 pounds; and effectively bear the weight of an average adult.
• A heart to serve patients and families and a passion for providing the best possible care.
