Social Worker (MSW)

About The Position

Agape Care Group is seeking a hospice medical social worker to support patients and families during the end-of-life journey. The role provides psychosocial evaluations and ongoing counseling in accordance with each patient’s plan of care, along with emotional support when it is needed most. The position is part of a team focused on compassionate care and meaningful experiences for patients facing life-limiting illnesses and their families. Requirements

A heart to serve patients and families and a passion for providing the best possible care.
MS degree in social work from an accredited school of social work approved by the Council of Social Work Education.
Current state license as a social worker.
At least 2 years of clinical work experience; healthcare or hospice experience is preferred.
Reliable transportation.
Ability to sit, stand, bend, and move intermittently, and to lift at least 25 pounds and bear the weight of an average adult effectively.
