About The Position

The Social Worker CM investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. This is an onsite, full-time position in the GH Care Management department at UCHealth Greeley Hospital in Greeley, Colorado. The schedule is 80 hours per pay period on day shift. Pay is $31.61–$47.41 per hour, depending on relevant experience. UCHealth offers a career ladder through which employees can develop skills and responsibilities and advance in pay.

Requirements

Master's degree in Social Work.
State licensure as a Licensed Social Worker (LSW) or Social Work Candidate (SWC).

Responsibilities

Interview and assess patients and/or their family members, caregivers, and legal representatives.
Determine and prioritize needed services and interventions, and provide or arrange them internally or externally.
Participate in case reviews to evaluate case management and progress.
Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
Assist with discharge planning and processes.
Identify appropriate resources, including transportation, housing, healthcare, and social or spiritual services, and provide referrals as part of discharge planning.
Supervise and/or train new staff, students, and interns.
Use critical thinking and decisive judgment, work with minimal supervision, and take appropriate action in a fast-paced environment.
