Part Time Social Worker - Eddy Visiting Nurse & Rehab
About The Position
Eddy Visiting Nurse & Rehab Association is hiring a part-time Social Worker to manage a caseload of patients who need social work support through in-home visits. Some patients and families also receive virtual or phone-based support, either instead of in-home visits or alongside them. The role includes collaborating with an interdisciplinary care team and helping patients remain at home or transition to a higher level of care when needed. After training, the social worker can start and end the workday from home and follow a flexible schedule. The organization provides a comprehensive home health care orientation, social work supervision, computer training, assistance obtaining insurance authorizations, and support staff. Applicants may also shadow an experienced Eddy VNRA social worker.
Requirements
- NYS Social Work Licensure.
- At least 1 year of relevant experience in home care, the medical field, work with seniors, or a related area.
Responsibilities
- Manage a caseload of patients in their homes.
- Work with an interdisciplinary care team of nurses, therapists, and aides.
- Conduct comprehensive assessments of patient and family needs.
- Connect individuals with community resources to help them remain in their homes or move to a higher level of care when necessary.
- Provide social work support through in-home visits and, where appropriate, virtual or phone-based support.
